About This Grant
The John A. Hartford Foundation (JAHF) is seeking proposals from organizations that have not previously received a major grant from the foundation for national or multi-site initiatives aligned with at least one of JAHF's three priority areas: Age-Friendly Health Systems, Family Caregiving, and Serious Illness and End-of-Life Care. Up to 10 grants will be awarded, each up to $300,000 over two years, through a two-stage process (letter of intent followed by full proposal).
